Home Health

Home health care is intended for clients that are well enough to be discharged home, but still require skilled nursing personnel to assess, initiate and oversee nursing interventions. Unlike other home health services, the Supportive Health Group Home Health team includes Nurse Practitioners and MDs who can provide immediate medical care and prescriptions when your PCP is unavailable.

Nurse Call Light

Supportive Health Group created the Nurse Call Light app for the ability of patients to leave the hospital while keeping the same security of immediate communication with their nurse 24/7. Just like the nurse call button in the hospital, our attention is at your command—even from home.

The Nurse Call Light App is available to all of our patients and families.

Hospice & Palliative Care

Supportive Health Group is a leading provider of hospice and palliative care services in Southern California. Our team works closely with the patient’s family and physician to meet the patient’s needs in the comfort of their own home, or in the residence of their choice. Supportive Health Group takes a progressive approach to hospice nursing and care, focusing our energies on bringing this gift to more eligible patients and developing new ways to better serve the patients who entrust us with their care.

Remote Monitoring

Patients with serious health issues need medical answers when they need them. Immediacy is our goal to improve care and comfort. Supportive Hospice Care provides an Apple Series 7 or Samsung Galaxy 4 Watch for eligible patients to wear for remote monitoring of their overall health.
